Primary Square, Inc. is a software company. We build and operate the Primary Square platform: your health-record vault, HOPE (your AI health companion), and the marketplace where you find and work with independent clinicians.
One account, one bill. Everything you pay on Primary Square runs through your Primary Square billing portal, your platform subscription, your clinician's membership fee, and any storefront purchases, itemized on a single monthly bill, charged to the payment method you choose.

Your clinician's membership fee. Set and published by your chosen clinician before you commit. It appears on your Primary Square bill and is paid to your clinician through the platform; a price change requires notice to you before it takes effect.
Storefront purchases. Non-prescription items, supplements, programs and add-ons, offered by independent clinicians are purchased through the same checkout. The price you see is the price you pay; Primary Square retains a disclosed 3% platform transaction fee plus card-processing costs, with the rest going to the clinician.
Where medical care fits. Primary Square, Inc. provides software, billing, and platform services. The clinicians on Primary Square are independent, licensed professionals who set their own fees and make the clinical decisions in your care. Prescribed medications are not sold through the Primary Square platform or its checkout.

By subscribing you authorize Primary Square, Inc. to charge your payment method $25.00 each month until you cancel. Cancel anytime in Settings, then Billing; your access continues to the end of the paid period and no further charges occur. Charges appear on your statement as PRIMARYSQUARE.COM. Primary Square, Inc. is a software company. We build and operate the Primary Square platform: the health-record vault, HOPE (an AI health companion), and the marketplace where patients find and work with independent clinicians.
Everything paid on Primary Square runs through the Primary Square billing portal, the platform subscription, the clinician's membership fee, and any storefront purchases, itemized on a single monthly bill charged to the payment method the patient chooses. At launch, marketplace payments are processed through the platform using Stripe Connect, with independent licensed clinicians as connected accounts and disclosed platform fees.

We answer email within one business day. Email is the fastest route for anything involving your records, your billing, or a request to close your account.

Retrieving your medical records, storing them in your vault, and being matched to a clinician are free, and stay free whether or not you ever choose a clinician.
If you join a clinician's practice through Primary Square, we charge a platform fee of $25 per month. That fee is billed by us, to you, and covers the software your clinician uses to care for you.
Your clinician sets their own membership price, which is separate from our platform fee and is paid to the clinician, not to us. We never mark up a clinician's price and we never take a percentage of it.
Signing up is free. There is no subscription and no setup fee, and nothing is charged until you have patients.
Once you are live, patients pay us the $25 per month platform fee directly. We do not take it out of your membership fee.
Standard credit card processing fees apply to payments you collect. A 3% transaction fee applies to goods and programs sold through your storefront, such as supplements and add-on programs. It never applies to your membership price.
All amounts are in US dollars and exclude any taxes that may apply. We will tell you in writing at least 30 days before any price change takes effect.
Platform fees are billed monthly in advance. Because the service is delivered continuously across a billing period, we do not prorate partial months.
If you are billed after cancelling, or billed in error or twice, we refund the charge in full. Contact us and we will process it within 5 to 10 business days to your original payment method. If you subscribed but have not used the platform, contact us within 30 days of the charge and we will review a refund of that charge.
Platform fees are billed monthly in advance. Because the service is delivered continuously across a billing period, we do not prorate partial months.
Membership fees are set by your clinician, and refunds of those fees are governed by the clinician's published refund terms. Because all billing runs through Primary Square, start with us: contact support and we coordinate the refund with your clinician and process any approved amount back to your payment method.
Before disputing a charge with your card issuer, please contact [email protected]. Most billing issues are resolved within one business day. If you do open a dispute, we will provide your payment provider with our records of the transaction and the service delivered.
Charges from Primary Square appear on your statement as PRIMARYSQUARE.COM.
You can cancel at any time, from your account settings or by emailing us. There is no contract, no minimum term, and no cancellation fee.
Cancellation takes effect at the end of the billing period you have already paid for. You keep access until then, and you are not billed again.
Your records are yours. After you cancel you can export your complete vault for 90 days. After that we retain your record only as long as the law requires, and you can ask us to delete it at any time.
You can close your practice on Primary Square at any time by giving us 30 days written notice, so that your patients can be told and their care can be transitioned properly.
You keep your patient relationships and your clinical records. We will provide an export of your practice data on request.
We may suspend or close an account for non-payment, for a breach of these terms, or where we are required to by law or by our financial partners. Except where the law or a suspected fraud investigation prevents it, we will give you 30 days notice and a chance to export your data.
By using Primary Square you agree to these terms. If you do not agree, please do not use the service.
We provide software and a marketplace. We are not a healthcare provider, we do not employ clinicians, and we do not practice medicine. Clinicians listed on Primary Square are independent, hold their own licenses, carry their own insurance, and set their own fees and clinical policies. Your care is a matter between you and your clinician.
We are not an insurer and we are not a health plan. Nothing we offer is insurance, and paying us does not cover the cost of care.

You must be at least 18 and able to enter a contract. You are responsible for the accuracy of the information you give us and for keeping your login credentials secure. Tell us promptly if you believe your account has been accessed without your permission.
When you ask us to retrieve your medical records, you authorise us to request them from providers, pharmacies, laboratories and health information networks on your behalf. We act on your instruction. You can withdraw that authorisation at any time.
Records are assembled from third-party sources and may be incomplete, delayed, or contain errors made by the original provider. We do not verify their clinical accuracy. Always confirm anything that matters with your clinician.
Parts of the service use automated systems to summarise records, surface patterns and draft documents. Output is a starting point for a qualified clinician, never a diagnosis, and never a substitute for professional judgment.
Do not use the service unlawfully, do not attempt to access another person's records, do not scrape or reverse engineer the platform, and do not upload malicious code. We may suspend access to protect the service or the people on it.
The Primary Square name, logo, software and site content belong to us. Your health records and your clinical notes belong to you and your clinician.
The service is provided as is. To the fullest extent the law allows we disclaim implied warranties, and our total liability for any claim relating to the service is limited to the fees you paid us in the 12 months before the claim arose. Nothing here limits liability that cannot lawfully be limited.
We may update these terms. If a change materially affects you, we will give notice by email or in the product at least 30 days beforehand. Continuing to use the service after a change takes effect means you accept it.
These terms are governed by the laws of the Commonwealth of Virginia, without regard to its conflict of laws rules. Disputes will be brought in the state or federal courts located in that jurisdiction.
